VITAL RECORDS
Birth/Death Certificates are available for those born/died in Missouri at any local public health department during normal business hours. There is an application and fee for each certified copy. You can find the form at this link BIRTH/DEATH Application You must show photo ID or bring two pieces of mail with your name and current address.
Appointments
We prefer you make appointments for nursing services. That ensures that we have nurse available and time to fill your request.
Appointments are REQUIRED for women’s health services and WIC.
You can call 417-345-2332 ext 100 to make appointments or be transferred to the appropriate department.
Local Ordinances
Dallas County Health Department does have three local ordinances in place.
Any person selling food for human consumption must have an inspection and a permit.
All housing units must have a proper working onsite wastewater system. New contruction, repairs and replacements are required to be done by a registered onsite wastewater installer and have a permit.
We respond to emergencies involving food. Fire, tornado, power outage, truck wrecks are examples of our emergency response.
